About Community Health Centres

The services at Community Health Centres (CHCs) are designed to help people in a community get health care and attend programs regardless of their life situation or financial status. CHCs promote and support basic elements of life that benefit our health, like dignified work, a clean environment, affordable housing, access to food, and good education.

CHCs offer a range of primary health care services and programs that promote better health in diverse communities across Ontario.

Community Health Centres are not-for-profit organizations and are governed by community boards.

As a CHC, we follow the Model of Health and Wellbeing. When we look at your health, we view it as a whole piece rather than only physical health. Health does not simply mean the absence of being sick – it’s about feeling good physically, mentally, socially and spiritually. Attributes of the Model of Health and Wellbeing.
