We actively review any complaints submitted at regular intervals with our management team and our Board of Directors. An action plan for follow up and mitigation is created to ensure adequate steps are taken for correction and prevention of recurrence[a1] .

Every attempt is made to resolve a conflict or complaint at Four Villages with our clients and/or staff and make our letter for complaints available to our clients and community members. However, if a situation arises that cannot be solved at Four Villages, there are other options available. You can contact the Patient Ombudsman of Ontario or the Information Privacy Commissioner of Ontario and file a complaint.
