About West Toronto Community Health Services (WTCHS)
Regeneration Community Services, Storefront Humber Inc. and The Four Villages Community Health Centre have amalgamated. We are a single agency for community health and support services. Our three organizations are now one legal entity under the legal name of West Toronto Community Health Services (WTCHS).
Our community health agency now provides our clients and communities with the greatest range of community health and support services. This makes us a distinctive agency in Toronto offering a unique blend of community support, community mental health and addictions services, primary health care and home care.
As a single agency we are registering under a legal name of West Toronto Community Health Services. We will continue operating as Regeneration Community Services, Storefront Humber and The Four Villages Community Health Centre.
We are community governed. We continually look for ways to serve our clients and our community better and to provide enhanced access to equitable care. Through this amalgamation, we can build stronger teams of community health providers. By working together, we can offer our clients, and clients of partner organizations, a more integrated care experience and better health outcomes.
Why the Three of Us?
Our services are complementary and help meet our clients’ needs to be healthy and stay in the community.
- Regeneration Community Services provides case management services, supportive housing and peer support programs for people living with mental health and addictions
- Storefront Humber offers a broad range of community support services to seniors and adults living with disabilities. Storefront is also a homecare service provider organization with Home and Community Care Support Services
- The Four Villages Community Health Centre provides health care services and health promotion programs through an inter-professional, team-based primary health care model
We have a shared history of small beginnings and we have each grown to serve our diverse communities better and more equitably. By coming together as a single organization, we are transforming how we work toward our shared missions.
