Community health workers support individuals and families who need help finding or getting a service and can’t do it themselves, for eg.:

• Helping people find their way through systems that can be complicated including calling agencies to get services
• Filling out forms (Ontario Disability Support Program, Ontario Works, Ontario Health, insurance, financial)
• Providing programs and workshops that promote health
• Offering counselling to help cope with life’s problems

Our community health workers help people in our community with many different types of challenges by offering individual and group support.
